Rebuilding telehealth intake so patients actually finish it

A 34-field medical intake form was losing four in ten patients before their first appointment. I redesigned it as a conversational, save-as-you-go flow.

Problem

Shadowed 9 patient intake sessions; mapped where hesitation turned into abandonment — mostly insurance and history fields.

Process

Prototyped three flows, tested with 14 participants, and cut required fields from 34 to 12 with progressive disclosure.

Outcome

Completion rose from 58% to 83% in the A/B test; the pattern became Fernwell's default for all clinical forms.
